Please find below the process and necessary forms we require for a cremation.

Contact Twentymans

The first step to arranging a cremation with Twentymans is to speak to our trained and experienced staff. 
At the moment this should be done by phoning 0800 500 003.

Filling in the forms

There are a number of forms that must be filled in and signed.
These include the permission for the cremation and the registration of death to Births, Deaths & Marriages.

COVID-19 - Level 2 Forms


Funerals and Tangihanga with between 11 and 50 people can happen IF the Funeral Directors is advised in advance and gains agreement from the Ministry of Health by completing a Registration of Intended Service Form and a Contact Tracing Form is used a the Funeral or Tangihanga.
